A hypermature cataract with intumescence poses a great challenge to make a successful capsulorhexis. Many times, the initial niche into the anterior capsule causes sudden and complete extension of the tear which is classically known as ‘Argentinian Flag sign’. The principal reason behind this is the high intra-capsular pressure due to swollen and sometimes liquified cortex. A solution is to decrease the intralenticular pressure by controlled egressing of the fluid and cortex so as to enable a capsulorhexis later. This is achieved by an ‘Inverted T technique’ using a 26G needle, micro scissors and Utrata forceps. It has a high success rate.
