A 25 year old female presented to the outpatient clinic with complaint of redness and swelling nasally in right eye since four days. She reported swelling of the right upper eyelid one week preceding these symptoms followed by mild swelling and redness temporally in this eye.Slit lamp examination revealed an elevated cystic lesion beneath the conjunctiva nasally with a cord-like structure visible through it. On applying gentle pressure through the lower lid, swelling showed undulations on its surface and the cord-like structure showed movement.A provisional diagnosis of a worm in the subconjunctival space was made and patient was taken up for its extraction. A conjunctival incision was given near one end of the worm and it was gently pulled out using two non-toothed forceps.It was white, thread-like, showing active motility and measured 12 cms. It was identified as Dirofilaria repens and is a very rare presentation.This video shows the procedure of extraction of the highly motile worm.
