Pterygium is considered an “Ophthalmic Enigma” by ophthalmologists worldwide. Conjunctival autograft (CAG) is documented ‘gold standard’ surgical option for pterygium surgery. Over the past decade, debate among ophthalmologists has centered whether to use sutures or sutureless options like fibrin glue to affix CAG. Through this video presentation, we propose a newer surgical technique – Releasable single suture technique (RSST) for pterygium surgery with a view of combining advantages of suture and sutureless cosmesis. A limbal based,thinnest tenon-free CAG was affixed by single 10-0 nylon suture tied in releasable fashion. On first post-operative day, under topical anaesthesia,suture was released on slit-lamp. 150 patients were operated with RSST, with 5%recurrence over 1 year follow-up.Thus, RSST may offer a good alternative for pterygium surgery with comparable success rates without suture-related complications.
