Anterior segment surgeons commonly manage cataractous lens subluxation. Methods of capsular support include various types of capsular tension rings and capsular hooks. We encountered a case of posttraumatic rhegmatogenous retinal detachment (RRD) with 6-clock hr temporal zonular loss. The RRD required a vitreoretinal procedure and we decided on combining the surgical procedure with management of cataractous subluxated lens. We present this video to demonstrate the nuances of combined surgery for subluxated lens and VR surgery. The difficulties encountered in capsulorrhexis, capsular support with iris hooks, phacoemulsification, IOL and CTR placement, managing a stable anterior chamber during vitrectomy and postoperative gas tamponade are described. This video will help prepare surgeons in planning a similar combined approach.
