In certain situations intraocular lens (IOL) turns opaque causing visual disturbances. In this video we showcase two cases of opaque IOLs which were replaced with clear IOLs. First case had undergone cataract extraction with hydrophilic IOl few years back, subsequently he underwent vitrectomy with gas for retinal detachment. Patient presented with hazy vision and opaque IOL few months later. We explanted the IOl and exchanged it with posterior iris fixated IOL. Second patient had also presented with opaque IOL one year post cataract extraction and hydrophilic IOL implantation which was exchanged with posterior chamber multipiece IOL in sulcus. Both the IOLs underwent electron scanning microscopy revealing calcium and phosphate deposits. One of the opaque IOLs also revealed calcium on von kossa staining. There was improved visual acuity and patient satisfaction in both the cases. Thus this video illustrates different way of surgical management of opaque IOLs.
