Purpose: This video demonstrates the surgical pearls in the procedure of the Modified Yokoyama procedure, it’s indications and outcome. Video : 22 year old lady with LE vision of 2/60 and refractive error of -15.50 and axial length of 30 mm. RE showed -5.75 with vision improving to 6/6. She had a 50 PD left esotropia with 15 PD left hypotropia. Her LE abduction was limited by -3. MRI findings suggestive of myopic strabismus fixus. Modified Yokoyama procedure(Suture Loop Myopexy), was performed along with a medial rectus recession for a tight MR. This video demonstrates the steps of the surgery along with surgical pearls and patient selection and post-op pitfalls. Postoperatively, she was 6 PD left exotropia, with excellent cosmesis. Conclusion: Suture Loop Myopexy is an effective and reversible procedure, that can be tried in the treatment of myopic strabismus fixus. Selection of cases and procedure are of importance. Surgical pearls and pitfalls are discussed.
