Purpose- To demonstrate the surgical technique of lateral rectus deactivation in a seven year old boy with congenital third nerve palsy
Methods- A seven year old boy presented with his right eye fixed in abduction since birth. He had mild ptosis and all other extraocular movements were restricted. He could fix using that eye with a face turn to the opposite side.
A surgical correction of his squint was sought where in a procedure of lateral rectus fixation to the periosteum was done. This was combined with medial rectus resection along with nasal transposition of the superior oblique to the medial rectus (Scott`s procedure).
Results- Post operatively the child was orthophoric at 1 year follow up along with attainment of binocular single vision at near and gross stereopsis.
Conclusions-Lateral rectus deactivation can be considered as a good option for managing large angle exotropia in patients with third nerve palsy.
