An eye with severe microphthalmos and coloboma poses many challenges for cataract surgery such as crowded anterior chamber, insufficient pupillary dilatation, large nuclear cataract, inferior zonular dehiscence, small capsular bag with lack of suitable-sized smaller IOL. Two cases of such a difficult surgery are demonstrated with tips for a successful outcome. The video highlights innovative modifications to the nucleus delivery technique and IOL implantation to improve the results.
