Phacoemulsification procedure is getting safer and safer with advent of new machines and trained surgeons. Still occasionally complications occur even with best of surgeons and simplest of cases. Most of the intraocular lenses are designed to be placed in the bag and any IOL placed outside the bag carries significant risk of other complications like persistent inflammation, irritation, retinal detachment, cystoid macular edema,raised intraocular pressure, suboptimal visual outcome etc. Certain customised premium IOLs like multifocal, toric and toric multifocal have not many options but to be put in the bag for optimum outcomes. Placing IOL in the bag despite rent is possible if we follow certain rules. In this video we will demonstrate pearls of placing IOL in the bag despite posterior capsular rent.
