Capsulorhexis in total fibrous anterior capsule is always a challenge. Attempts to use micro scissors and forceps result in frequent extension of the tear to the posterior capsule with disastrous consequences. This is a new technique where cutting of the capsule is done in a very controlled way with a phaco probe using moderate ultrasound power, vaccumm and flow rate. There is a similar technique used in intumescent cataract where initial opening is made with phaco probe to be subsequently completed with forceps. Our technique is different from it in the sense that whole capsulotomy is completed with phaco probe cutting only.
