Submacular hemorrhage is a common manifestation of diseases like AMD, IPCV, trauma. The natural history of untreated, thick submacular hemorrhage is generally poor, probably due to a combination of the toxicity of subretinal blood, as well as the underlying pathology causing the hemorrhage. Over the past two decades, a variety of treatment options have become available. However, the results of these interventions are often disappointing, and the management of submacular hemorrhage remains a therapeutic challenge. Apart from the underlying pathology, other factors that affect prognosis include increasing size, thickness and duration of submacular blood. This video describes indications of surgical intervention and which intervention, depending on size, thickness and duration. The video shows the stepwise techniques of various interventions i.e. only pure gas, intravitreal injection of tPA and expansile gas, vitrectomy and tPA (intravitreal and subretinal) done.
