Posterior capsular rent with posterior dislocation of nuclear fragments is a recognised complication of cataract surgery including Small-incision cataract surgery (SICS). This can lead to inadequate or complete loss of capsular support making posterior chamber Intraocular Lens implantation a difficult proposition.
In these situations, we are left with options of Scleral-fixated PCIOL (Glued/sutured) or Iris-fixated/Iris-supported anterior/posterior chamber IOLs. Retrofixation of Iris-claw IOL to the iris is a viable option in such a situation with intact iris structure. I present my technique of Retrofixation of Iris-claw IOL through scleral tunnel already created at the time of SICS. Special instruments required for Iris-claw IOL fixation will be highlighted. The procedure is usually performed after anterior vitrectomy or parsplana vitrectomy for removal of dropped lens fragments. The technique can also be used as a secondary procedure in cases of Aphakia without capsular support.
