Deep stromal opacities of corneal are dealt with deep anterior lamellar keratoplasty(DALK). This maximizes the usage of corneal tissue whose demand and supply has a huge gap. Sometimes the opacities are associated with cataract that has to be removed. Phacoemulsification with DALK is a skilled surgery and requires lots of patience. The video will highlight the intricacies of DALK. It will also highlight the cataract removal by phacoemulsification emphasizing the various precautions and modifications in fluidics. The video will showcase a rare complication during DALK where the descemet’s membrane suddenly gave away at the trefined margins. This resulted into sudden vertical gush of fluid resembling a fountain. The management of this complication will be showed at the end of the video.
