Purpose – To show a challenging video where surgeon had taken an alternative pathway as complication occurred
Methods –42 years old male patient presented with traumatic cataract with 1200 subluxation inferotemporally. Capsulorrhexis was done. Three capsular hooks were applied at the site of subluxation before carrying out an uneventful phacoemulsification. 10-0 prolene suture was passed through the sclera and was taken out through the main port. Suture was tied with Cionni ring. Cionni ring was inserted but could not be placed in the bag and during maneuvering, the anterior capsular margin was breached. So explantation of Cionni ring was done with difficulties. Posterior capsulorrhexis was done and posterior optic capture of MP PCIOL was done.
Results – Even after one year PCIOL was centered with BCVA of 6/6.
Conclusions – Posterior capsulorrhexis with optic capture of PCIOL can be a good alternative in subluxated cataract where anterior capsule is breached.
