This video shows an unusual case of Combined Retinal Detachment ( CRD ) with Posterior closed funnel type with no view of disc due to Fibrovascular Proliferative ( FVP ) tissue covering the posterior pole and a surprise large hidden break beyond the inferior arcade. The Posterior hyaloid was first separated temporal to the macula and then separated all around. The FVP was dissected and removed from the Optic nerve head and surrounding retina with a forceps and trimmed with cutter. On examining the periphery a large break was noted inferior to the arcade, covered with membranes. This break was opened and a large Relaxing Retinotomy was done. Membranes dissected above and below the retina en masse. Fluid gas exchange was done. Retina settled satisfactorily and endolaser was done around the break along with PRP. Silicone oil injected and ports sutured.
It was a rare case of CRD with Posterior Closed Funnel configuration and a GRT type hidden break in Proliferative Diabetic Retinopathy.
