Aim: To display comprehensive eye care model for community services which will comprehensively all aspects with proper documentation.
Method: On arrival of patients are passed through a chain system where separate rooms for history ,vision, IOP, Sac syringing and cycloplegic refraction, all rooms have a computer with EMR. After all tasks are complete comprehensive examination and verification done by ophthalmologists including slit lamp and indirect ophthalmoscopy. After examination patients are divided in categories cataract surgery, optical services, medical treatment and follow up. All patients who have other ailment are referred to respective departments for further management. All patients with glaucoma,retina,cornea,lacrimal,Oculoplasty,squint uveitis are diagnosed during single visit and after completion of other problems cataract surgery may be performed.
Conclusion: Complete eye examination is performed with proper documentation with this model.
