Uveitis treatment is asynonymous with steroid bombardment. Steroids being a double edged sword can backfire if injudiciously used in viral retinitis, tubercular choroiditis, toxoplasma retinitis. Avoiding them altogether can cause so many perils and are the lynchpinn in tackling paradoxical reactions. The appropriate titration of inflammation with appropriate anti-infectives and steroids is the mantra of treatment success. The USP of any uveitis specialist is extending the armemantarium of uveitis treatment from beyond steroids to immunosuppressives with its choices. Where steroids are ineffective in uveitis is an eye opener to a budding ophthalmologist. Limitation of steroid use with co-morbidities without undertitration is a tight rope walk. Dynamic assessment of response to steroid treatment is the fulcrum to the astute clinician to clinch several subtle diagnosis.
